However, a recent scoping review published in BDJ Open sheds light on a shadowy corner of the internet: the rampant spread of oral health misinformation. From viral DIY dentistry hacks that promise a “perfect smile” to long-standing, scientifically debunked myths about fluoride, the online landscape is flooded with content that is often misleading, unverified, or outright false. While we are aware that we shouldn’t believe everything we read, this study highlights how effectively algorithms push sensationalist, aesthetic-focused advice over credible, evidence-based guidance, leaving the average user to navigate a minefield of potentially harmful dental advice.
The scale of this issue is significant because oral health content on platforms like Instagram, TikTok, and YouTube often sits in a legal gray area. Many viral dental products—such as whitening kits or specialized toothpastes—are categorized as cosmetics rather than medical devices in various jurisdictions. Because of this, they are subject to far less stringent regulatory oversight than clinical treatments, allowing influencers to make bold, unsupported promises about results. The researchers found that engagement metrics tell a concerning story: the content that gets the most likes, shares, and views is rarely the most scientifically accurate. Instead, the platforms prioritize visually stimulating, high-energy videos that might feature exaggerated credentials or clinical inaccuracies, effectively drowning out the voices of verified dental professionals.
Perhaps the most fascinating—and somewhat surprising—finding of this review is the gap between the prevalence of misinformation and our understanding of how it actually affects human behavior. While it is easy to assume that watching a “DIY root canal” video or reading a post that claims fluoride is toxic would drive people away from professional dentists, the scientific evidence is surprisingly thin. Out of the 61 studies examined, only a small fraction directly measured whether this misinformation caused patients to delay treatment, feel increased anxiety, or lose trust in their dentists. Currently, the impact of online misinformation on real-world dental health remains largely a plausible hypothesis rather than a proven clinical crisis. We know the misinformation exists, but we are still missing the smoking gun that links a specific viral post to a patient’s direct physical harm.
The thematic focus of this online misinformation is as diverse as the internet itself. Fluoride and water fluoridation remain the most frequently debated topics, consistently appearing in academic literature as hotbeds for conspiracy-leaning content. However, the rise of aesthetic dentistry has shifted the focus toward cosmetic interventions. From DIY aligners that promise a straight smile without an orthodontist’s supervision to influencers promoting “bio-hacking” dental implants, the content is becoming increasingly sophisticated. The study notes that YouTube is the most heavily researched platform, likely because its long-form nature allows for more detailed, though often misleading, health narratives, while TikTok is quickly becoming the home for quick-fix, aesthetic-focused dental hacks that can gain massive traction in hours.
The review also points to a glaring need for a more standardized approach to how we study this digital phenomenon. Currently, researchers are using a wide variety of definitions for “misinformation,” “poor quality,” and “lack of transparency,” which makes it difficult to compare findings or build a coherent strategy for combatting these issues. Furthermore, the populations being studied are often narrow, meaning we don’t have a broad picture of how different demographics—such as younger, social media-native generations versus older adults—are being differently influenced by these digital trends. We are essentially trying to measure a moving target, as algorithm updates, platform features, and user behaviors change faster than the academic community can publish their findings.
Moving forward, the authors of the review emphasize that we cannot simply “fact-check” our way out of this problem. If the dental community wants to reclaim the space, it must learn to speak the language of social media. The recommendations are clear: health organizations need to move beyond static, boring web pages and start creating evidence-based content that is just as engaging and optimized for algorithms as the misinformation they aim to counter. This requires a systemic shift that includes tighter regulation of cosmetic dental products, more active monitoring by professional associations, and a concerted effort from dental professionals to become active, relatable voices online. By meeting patients where they are—on their screens—dentists can begin to bridge the gap between digital noise and clinical reality, ensuring that the next time someone searches for dental advice, they find something that will actually keep them smiling.
