The recent statement from Healthcare Workers for Palestine (HW4P) serves as a sobering reminder that xenophobia is far more than a political talking point—it is a pervasive public health crisis. When we allow hatred, intimidation, and isolation to take root in our society, the consequences are immediate and devastating. Beyond the obvious physical and psychological trauma inflicted upon refugees and migrants, this climate of hostility compromises the integrity of our medical systems. By disrupting essential disease prevention and treatment programs, xenophobia doesn’t just harm the “outsider”; it destabilizes the health and cohesion of the entire nation, creating dangerous gaps in the safety net meant to protect us all.

At the heart of this conflict lies the dangerous wildfire of misinformation. In South Africa, constant, unchecked claims regarding the number of migrants, their legal standing, and their perceived strain on public services have clouded the public discourse. These narratives are often presented without evidence, yet they are used to justify hostility. When people are led to believe that their own lack of access to care is caused by the presence of a neighbor from another country rather than systemic failures, social fractures deepen. This environment of suspicion serves as a convenient distraction from the real challenges we face, turning common citizens against one another while the true sources of our socio-economic struggles remain unaddressed.

Perhaps most disturbing is the direct interference we are seeing in medical care. Despite clear constitutional protections and explicit court orders, organized groups have increasingly blocked migrants from entering hospitals and clinics. This is a fundamental violation of human rights and medical ethics. A hospital should be a sanctuary—a place where a person’s status is irrelevant compared to their need for healing. When we allow the doors of a clinic to be barred to someone based on their nationality, we abandon our commitment to “do no harm” and betray the very essence of public health. This obstructionism creates a climate of fear that keeps the most vulnerable from seeking help, even in life-threatening emergencies.

The response from authorities has, thus far, been deeply concerning. Law enforcement, which is tasked with upholding the rule of law and protecting the most vulnerable, has frequently been slow to act on court orders intended to ensure safe passage for refugees. This inaction does more than just leave migrants exposed to violence; it signals that harassment is acceptable, potentially emboldening those who seek to cause harm. When the state fails to step in to protect those who are targeted for who they are, the entire legal framework begins to erode. Justice is not a selective privilege; it must apply to everyone living within our borders, or it ceases to be justice at all.

Furthermore, the bureaucracy has failed to provide the necessary clarity to handle these complex situations. Health departments at both national and provincial levels have left healthcare workers in a state of limbo, lacking the clear, consistent policies required to navigate the intersection of migration and patient care. This void of leadership leaves frontline nurses, doctors, and administrators vulnerable, as they are forced to make life-or-death decisions without institutional backing. Without a unified, compassionate approach from the top down, the uncertainty only multiplies, leaving both the healthcare providers and the migrant communities in a state of perilous isolation.

Ultimately, we must confront the reality of political scapegoating. History has shown us that when politicians choose to blame migrants for broad national grievances—like unemployment or infrastructure challenges—violence often follows closely behind. This is a tired, destructive pattern that treats human beings as pawns in a pursuit of favor. HW4P’s call for urgent action is not just a plea for migrants; it is a call for a more dignified, moral, and functional South Africa. We must reject the politics of exclusion, prioritize evidence over misinformation, and uphold the constitutional guarantee of healthcare for all. Only by treating every person with the respect they deserve can we begin to repair the social fabric that xenophobia has so effectively torn apart.
